As you begin at Jackson Elementary, we've made school registration as easy as possible. Simply read this page, print off the necessary forms, and complete. We hope to see you at our Kindergarten Open House and Registration on April 14 from 3:30-5:00 at our Primary Campus (4191 Northside Drive). There, we’ll help you register your child as well as sign up for Jackson's popular "Camp Jump Start" with your completed registration. Don’t forget to bring your “Primary Paw Package” School Supplies Form, New Student Questionnaire, Camp Jump Start Registration Form, Bus Information Form, Directory Information Form and Email Blast Information Form too. We look forward to meeting and welcoming your family to our fantastic school!

1 - APS Student Enrollment and Registration form

2 - Proof of Residency - Click here for complete list of what is considered satisfactory proof. Please plan on having at least the following but be certain to carefully read the attached list for additional items that may be needed:

• Driver’s License of parent OR guardian registering child
• Deed OR mortgage OR lease
• GA Power Bill
• Bank statement OR motor vehicle registration OR voter registration
• APS General Enrollment Affidavit OR APS Affidavit of Residency

3 - Certified copy of child’s birth certificate - Click here to learn how to obtain one.

4 - APS Immunization Requirements Form 3231 from your doctor - Click here for more information and a sample of Form 323. NOTE: Your child’s current form will be acceptable for Kindergarten registration purposes but you will be required to provide an updated form as of your child’s next birthday.

5 - Certificate(s) of Ear, Eye, and Dental Form 3300 from your doctor - NOTE: Your child’s current form will be acceptable for Kindergarten registration purposes. Also, you may need to obtain 3 separate certificates (1 for ear, 1 for eye and 1 for dental).

6 - Child’s original Social Security Card OR Statement of Objection to Use.

1 - Primary Paw Package - Instructional Materials and School Supplies are mandatory for each student and easy to purchase at the Open House!

2 - New Student Questionnaire - This helps with class assignments. Fill it out at home and bring it to the Open house.

3 - Camp Jump Start Registration Form - Sign your child up for this wonderful introductory camp when your school registration is complete.

4 - Bus Information Form - Help us plan for next year by bringing this form to the Open House.

5 - Directory Information and Email Blast Information Form - Help us stay in touch with you efficiently by bringing these forms to the Open House.

6 - Money/Checkbook - A lot will be available for convenient purchase at the Open House such as school supplies, registering for Camp Jump Start and kid-popular Jackson Spiritwear including a mandatory Kindergarten field trip t-shirt.


>> Please note the following:

** Only a parent or legal guardian will be able to register the student. The person who registers the student is the only person who will be allowed to withdraw the student.

** If you plan to register your child at the Kindergarten Open House at Jackson Primary please make a copy of ALL paperwork, ID’s, birth certificate, SS card, Immunization form etc. and bring the copies and completed originals with you to the Open House.

** No child can be registered for Camp Jump Start until your Kindergarten registration is complete.

** A notary will be available at the Open House to notarize affidavits free of charge.
